GP practices are working together with community, mental health, social care, pharmacy, hospital and voluntary services in their local areas in groups of practices known as primary care networks (PCNs).
PCNs build on existing primary care services and enable greater provision of proactive, personalised, coordinated and more integrated health and social care for people close to home. Clinicians describe this as a change from reactively providing appointments to proactively caring for the people and communities they serve.

Leicester, Leicestershire & Rutland ICB
Our practice is part of a local Integrated Care Board who plan and manage most local health care services.
Following a change to the Health and Social Care Act, primary care trusts (PCTs) were abolished and the new Integrated Care Board (ICB), led by GPs, took over the responsibility for planning and managing health care for their local population.
The ICB aim to improve the way the local NHS engages and communicates with our patients, public, clinicians, staff, partners and other stakeholders.
