New Patients

Before registering, please check that you live within our practice area by using our boundary map or speaking to a member of our reception team.

Information You’ll Need

To complete your registration form, please have the following details ready:

• Full name (including any previous names)
• Date and place of birth
• Previous address and postcode
• Current address and postcode
• Name and address of your previous GP practice
• NHS number (if known)

If you do not know your NHS number, your previous GP practice may be able to provide it.

GP2GP Record Transfer

When you register with a new GP practice or leave our practice, we use GP2GP to transfer your medical records securely and electronically.

This means your medical records can be sent directly between GP practices, helping your new healthcare team access important information more quickly.

Please note that GP2GP is used for permanent registrations and transfers only and is not available for temporary patients.
